The monthly report is typed up by hand from a defect log, a scrap sheet and an inspection folder that never quite agree with each other, and whoever assembles it spends days chasing the plant for figures. This workflow gathers defect, rework, scrap and inspection records for the period, reconciles them against production volume and shipped quantity, and calculates every rate the same way each month so two reports can actually be compared. Anything that cannot be reconciled, a missing inspection sign-off or a scrap figure with no batch behind it, is listed as an open gap with a named owner rather than quietly averaged in. The draft goes to the quality manager with the trend for each defect family, the largest three by cost of poor quality, and any line that moved sharply since last month. A separate cut for the customer and the auditor is generated from the same source. What a human still decides is the rate definitions, which defects earn a corrective action, and what is safe to tell a customer before the data is confirmed.
